SECTION I - INSTRUCTIONS

You can submit this form by mail to Vivid Seats Purchase Settlement, c/o Settlement Administrator, 1650 Arch Street, Suite 2210, Philadelphia, PA 19103, or you can file your claim below. The deadline to submit or postmark a claim is February 14, 2022.

Please carefully review the announcement regarding relief available to California Postponed and Rescheduled Event Ticket Holders as well as the Published Notice regarding the Settlement before filling out this form. Terms in this California Request for Cash Payment Form are defined in the Published Notice, the Settlement Agreement and/or the Court’s Final Order and Judgment, each of which is available on the Important Documents page, by calling 1-833-321-1231 or emailing info@TicketPurchaseSettlement.com.

SECTION II - OPTIONS FOR RELIEF

If you purchased a ticket(s) through Vivid Seats on or before April 1, 2021 to an event(s) that, at any time from September 29, 2016 through April 1, 2021, was postponed or rescheduled but is not yet cancelled and has not yet occurred, you resided in California at the time you purchased the ticket(s) and you currently hold the ticket(s) to the Postponed Event or Rescheduled Event, you have the option to:

(i) Retain the ticket to the Postponed Event or Rescheduled Event, and if the Postponed Event or Rescheduled Event is cancelled, you will automatically be entitled to a Credit or you can elect a cash payment from Vivid Seats, so long as you do so within twenty-one (21) days of receiving notice that the Postponed Event or Rescheduled Event has been cancelled.

(ii) Request to receive a cash payment equal to the price of the ticket(s) you purchased (inclusive of base price, fees and delivery charges, exclusive of any insurance charges and less any amounts paid with gift cards, store credit or loyalty credit) to the Postponed Event or Rescheduled Event, less any sums already paid or spent in credit, by postmarking or submitting this form online by February 14, 2022. In addition, to be eligible for a cash payment, regardless of how you submit this form, you must relinquish ownership of the valid ticket(s) to the yet-to-occur Postponed Event or Rescheduled Event by February 14, 2022. If you have an e-ticket(s), you must relinquish ownership of the e-ticket(s) by emailing them to RetailHoldings@VividSeats.com. If you have a physical ticket(s), you must relinquish ownership of the ticket(s) by mailing the ticket(s) to Retail Holdings, 225 W Randolph Street, Floor 30, Chicago, IL 60606. If you have a transfer ticket(s) that is housed in a third-party app (e.g., Ticketmaster app), you must relinquish ownership of the ticket(s) by transferring ownership of the ticket(s) to RetailHoldings@VividSeats.com through the third-party app. In submitting the California Request for Cash Payment Form and returning your ticket, you shall relinquish and no longer be entitled to any other relief under the Settlement Agreement, including any Credit you could use toward a purchase through Vivid Seats that you might have received if your event was cancelled.

If you purchased tickets through Vivid Seats on or before April 1, 2021 to more than one event that, at any time from September 29, 2016 through April 1, 2021, was postponed or rescheduled but is not yet cancelled and has not yet occurred, you resided in California at the time you purchased the tickets and you currently hold the tickets to the Postponed Event or Rescheduled Events, complete and attach an additional page for each such event for which you seek a cash payment.

SECTION V - CLAIM CERTIFICATION AND RELEASE

I have received notice of the Settlement as well as the additional relief provided in Paragraph 23 of the Court’s Final Order and Judgment, and I submit this California Request for Cash Payment Form under the terms of the Court’s November 1, 2021 Final Order and Judgment and the Settlement Agreement. I acknowledge that under the terms of the Court’s November 1, 2021 Final Order and Judgment, I am bound by any Court’s Final Order and Judgment and, upon the Effective Date of the Settlement, will release claims against Vivid Seats as set forth in the Settlement Agreement. I submit to the jurisdiction of the United States District Court for the Northern District of Illinois with regard to my claim for cash payment and for purposes of enforcing the release of claims. I acknowledge that all claims are subject to investigation, and any false claims may be subject to legal action. By submitting this form and receiving payment hereunder, I understand and acknowledge that by electing a cash payment, I am relinquishing, and will no longer be entitled to, a Credit from Vivid Seats toward the purchase of tickets to events through Vivid Seats, pursuant to the terms of the Settlement Agreement and the Court’s Final Order and Judgment.

To be eligible for a cash payment, I understand that, by February 14, 2022, I must return the valid ticket(s) to the yet-tooccur Postponed Event or Rescheduled Event to Vivid Seats and relinquish ownership of the ticket(s) (as outlined above).
